Canadian Work Leave Request Form

Under the Employment Standards Act associates are entitled to a variety of Work Leaves based on specific criteria. For more information about Canadian Work Leaves in Ontario, please see the leaves of absence section in the Employment Standards Act here. Leaves of absence are unpaid, however you may be eligible for Employment Insurance (EI) benefits. More information about benefits can be found here.

It is important to know that leaves of absence are not to be used to request sick days. If you are calling in absent for your shift, you are required to be reported through the call-in procedure specific to your assignment.

Please submit your Work Leave Request Form as soon as possible before the start date of your leave. Pregnancy and parental require a two (2) week notice before commencing the leave. If an employee must begin a leave before notifying The Job Shoppe due to an emergency situation, the employee must notify The Job Shoppe about the leave as soon as possible after its start. The Job Shoppe will not accept any other request methods, including phone calls, separate emails or verbal communications.

Once The Job Shoppe receives your request, a representative will review that request and will follow up with you to advise if your request has been approved or denied, and if any additional information or documentation is required.
